Custom Catalyst Application Profiles 2.4

Fixes:
- New Base 12.1 CAP3: Imported all my modifications to the new 12.1 CAP3
- L.A. Noire: Added profile for DX11 (needs testing)

How to install:
- Remove existing CAPs/CCAPs via windows uninstall manager DO NOT reboot
- Install CCAPs and reboot
